Logos offers our clients the ability to view, schedule, cancel, and even re-schedule appointments through our client portal. You may also view & pay any outstanding balances online.

If you do not see your desired provider listed or are unsure which provider to choose, please use our chat feature or call us at 817-812-2880. Alternatively, you can click the button below to request a new appointment.

Please remember you are not able to cancel or re-schedule any appointment within 24-hours of the said appointment via your patient portal. In order to make any changes please call our office.
There is a no-show fee of $80 that will apply for appointments cancelled within the 24 hour window.
